The commission’s report said the general manager, who was in 2022 awarded the Australian Corrections Medal for “integrity, commitment to duty and ongoing devotion to continuous improvement and staff development”, also took no action against the prison officer who falsified the observation record and “failed to take managerial action in respect of the level of compliance generally with the policy as to required observations”.
